Low Level Laser Therapy Versus Pharmacotherapy and Inter Occlusal Splint Therapy in Improving Myofascial Pain Disorder Syndrome

Purpose: The present study was carried out to evaluate and compare the clinical effect in a short term of three conservative treatment methods for myofascial pain disorder syndrome (MPD): pharmacotherapy, inter occlusal splint and low level laser therapy (LLLT). Subjects and Methods: The present study was applied on thirty (n=30) females patients which suffered from myofascial pain disorder syndrome (MPD).They were selected for this study with age ranged from 17-45 years. Patients were divided equally and randomly into three groups; Group I : were treated by LLLT, Group II were treated by oral appliance therapy and Group III were treated by pharmacotherapy. The outcome variables were pain score and the maximum interincisal opening (MIO). Pain score was assessed by using Visual Analogue Scale (VAS); both variables were assessed preoperatively, postoperatively at the first month weekly, three months, and six months. Results: VAS scores decreased and MIO increased gradually throughout the follow up durations at first, third, and six months after treatment in all groups (P < 0.05). Although there was a difference between groups I, II and III, however, the difference was not statistically significant. Conclusions: low level laser therapy can be considered as a suitable and non-invasive treatment alternative for MPD. Also it was effective, had showing promising results and can be used as treatment of MP. LLLT shows its effects in a short term duration so it can be considered as a treatment of choice.

Surgery-first Approach Using Virtual Setup for the Treatment of Dentoskeletal Deformities

Purpose: to evaluate the accuracy of Computer-Assisted Surgical Simulation (CASS) in the surgery-first treatment of Dentoskeletal Deformities. Materials and methods: Ten patients with different dentoskeletal deformities, were involved in this study. Preoperative clinical evaluation, photographs, study models, and 2D lateral cephalometry were adopted for orthodontic evaluation if the case is eligible for the surgery- first approach. MSCT (in DICOM format), and optical scanning of the study model (in STL format) were adopted and imported to MIMICS software for construction of virtual 3D composite model. The virtual surgical simulation was performed on the light of the preclinical evaluation and preoperative records, then the occlusal splints and surgical guides were designed to transfer the planned surgery to the reality. For evaluation of the virtual surgical simulation 3D cephalometric analysis for the planned and postoperative readings. Result: No significant difference between the planned and postoperative 3D cephalometric analysis. Conclusion: Surgery First cases needed thorough orthodontic and surgical planning. Virtual surgical planning with subsequent designed occlusal splints and cutting guides allow for accurate transfer of the virtual planning into reality.

Effect of Simultaneous Vomerine Flap and Lip Repair on the Palatal Dimensions in Unilateral Complete Cleft Lip and Palate Patients

Purpose: to evaluate the effect of simultaneous vomerine flap and lip repair on the palatal dimensions in unilateral complete cleft lip and palate patients. Patients and Method: this study included 12 patients complaining of primary unilateral complete cleft lip and palate. They were classified randomly into two groups: Group I: included 6 cases, in which simultaneous cleft lip and hard palate repair was done using single layer vomerine flap. Group II: included 6 cases, in which separate repair of cleft lip and hard palate was done. Syndromic patients and patients with previous trial of repair were excluded. Results: showed reduction in A-A` alveolar cleft width and C-C` anterior palatal width occurred in both groups, while E-E` posterior palatal width increased in group 1 and decreased in group2 and the reverse occurred in BZ antro-posterior palatal width. Conclusion: Vomer flap is an easy, safe technique, with less time consuming and provides complete healing with less fistula formation.

Purpose: This study was carried out to evaluate both clinically and radiographicly the use of 3D printed computer assisted patient’s specific scaffold seeded with ABMMCs in mandibular reconstruction. Materials and methods: The present study was conducted on eight patients who needed mandibular reconstruction. All patients in this study were subjected to mandibular reconstruction using 3D printed computer assisted patients specific scaffold using PLLA scaffold seeded with ABMMCs combined with osteoplastic materials of biological origin as Osteomatrix (LLC Konectbiopharm, Russia) which composed of natural collagen and hydroxyapatite with affinity-bound sulfated glycosaminoglycan (SGAG). All patients underwent preoperative cranial-maxillofacial CT scans, immediate postoperative and after six months for assessment of bone density and volume changes. Results: The reduction in bone density (17.4%) and bone volume (14.5%) from immediate to six months was statistically significant. There was an increase in bone density in the area near the autogenous bone of the patient than the area far from the autogenous bone of the patient. Conclusion: Using 3D printed computer assisted patient’s specific scaffold seeded with ABMMCs combined with Osteoplastic materials of biological origin in mandibular reconstruction is useful in small defects.

A Comparative Study for the Effectiveness of Chemiluminescence and Toluidine Blue in the Early Detection of Oral Cancers

Purpose: This study aimed at comparing the efficacy of chemiluminescence and toluidine blue in detecting oral dysplasias and malignancies in at an early stage. Material and methods: A total of 26 patients who had potentially malignant lesions were included in the current study. All lesions were examined using Vizilite TM chemiluminescence followed by painting them with toluidine blue biostain. Then, all lesions were biopsied to be the gold standard for assessing both the chemiluminescence and the toluidine blue. Results: The current study showed low sensitivity, but high specificity and positive predictive value of TB (57.15%, 60%, and 85.71% respectively). On contrary, were the results of Vizilite (85.71%, 25%, and 57.14% on order). Conclusion: Although biopsy is the gold standard to detect dysplasia, toluidine blue and Vizilite were valuable non-invasive and rapid adjunct techniques that aided in the clinical detection and screening of dysplasia. They might have a special value in the screening for the existence of new lesions in patients with previous oral malignancies.
